When you fill a prescription, submit a form on this website, or call, text, or write to us, ThriveRx Health Pharmacy may collect your name, contact details, date of birth, prescription and medication history, insurance and payment details, and the health information we need to dispense safely. If you take part in health coaching, a wellness screening, or a biomarker panel, we also collect the intake information, measurements, and results connected to that service. Our website forms capture whatever you type into them, including any notes you leave for the pharmacist.
We use your information to dispense medications accurately, counsel you and carry out medication therapy management, package and synchronize your refills, process insurance claims and billing, pursue prior authorizations and manufacturer copay assistance on your behalf, build the coaching and supplement plans you ask us for, send refill and pickup notices by phone or text, and arrange local delivery or curbside pickup. We do not sell your personal information.
ThriveRx Health Pharmacy is a HIPAA covered entity and is obligated to protect your Protected Health Information. We maintain the administrative, physical, and technical safeguards HIPAA requires, and our workforce is trained on those requirements. This website policy is not a substitute for our Notice of Privacy Practices, which sets out your rights over your health information in full and is available at the pharmacy on request.
We may share your information with prescribers and other providers treating you, with your health plan or pharmacy benefit manager to process a claim, with a manufacturer administering a copay assistance program you enroll in, with the laboratory that runs a panel you order, with couriers only as needed to deliver your medication, and where the law requires it, including reporting to the Ohio Automated Rx Reporting System (OARRS), the state's prescription drug monitoring database. We do not hand your information to unrelated companies for their own marketing.
Prescription records live in our pharmacy management system, with access limited to authorized staff. Information you send through this website travels over an encrypted connection, and paper records are shredded when we dispose of them. No method of transmission or storage is completely secure, so please do not put sensitive health details into ordinary email.
You may ask to see or receive a copy of your records, ask us to correct information you believe is wrong, ask us to restrict certain uses or disclosures, ask us to contact you at a particular number or address, and request a list of disclosures we have made. You may also complain to us or to the U.S. Department of Health and Human Services Office for Civil Rights, and we will not retaliate against you for doing so. To make a request, call 740-417-4600 or write to the address below.

We may revise this policy. When a change is significant we will update the effective date shown with this policy and, where it makes sense to do so, tell you directly. It is worth reading this page again before you submit anything through the website.
