Medication Therapy Management (MTM)

Medication Therapy Management (MTM)

One appointment where a pharmacist reviews every medication and supplement you take.

Text the pharmacy at 740-417-4600.

See what every medication you take is doing together

Most people collect prescriptions one at a time. A cardiologist adds one, a primary care doctor adds another, a specialist adds a third, and a supplement goes in the mix from a shelf at home. Each prescriber sees their own piece of the list. Nobody sees all of it.

Medication therapy management is the appointment where someone finally does. It covers everything you take, not one refill, and patients across Delaware book it the same way they would book a physical.

What the appointment looks like

Scheduled time

You sit down with the pharmacist instead of talking over a pickup line.

Everything on the table

Prescriptions, vitamins, and anything you take without a label go on one list.

Something to act on

You leave with a written summary you can hand to any prescriber.

Larecia Seibert, PharmD, owns the pharmacy and holds MTM certification, so the review is done by the same pharmacist you speak with at the counter afterward.

What a full list turns up

Two drugs doing one job

When prescribers work separately you can end up on two medications from the same class, and that overlap usually shows only when the bottles are lined up side by side.

Supplements count as medication

Fish oil, magnesium, herbal capsules, and daily vitamins can thin blood, block absorption, or move a lab value, so they get reviewed alongside your prescriptions.

Directions that drifted

A dose gets changed over the phone while the printed label stays the same, and what you actually take quietly separates from what the record says.

Cost you stopped mentioning

If a drug is priced past what you will keep paying, this is where a cheaper equivalent gets raised with your prescriber or a stalled prior authorization gets picked up.

Bring these with you

  • Every prescription bottle you have at home, including ones you stopped taking.
  • Vitamins, supplements, and anything you buy off the shelf.
  • Your insurance card.
  • The names of the doctors and specialists you currently see.
  • Any side effect or symptom you have wondered about but never asked.

FAQ

Frequently Asked Questions

Yes. A medication review is booked ahead rather than done at the counter, because the pharmacist sets aside time to go through your full list without interruption. Call the pharmacy and pick a time that works. If you have a question about a single prescription, that does not need an appointment at all.

Medicare Part D plans are required to offer medication therapy management to members who meet the plan's criteria, which usually involve several chronic conditions, several covered drugs, and annual drug spending above a set amount. Eligibility is decided by your plan, not the pharmacy. Bring your card and we can check where you stand.

The pharmacist reviews, flags, and recommends. Your prescriber writes the change. If the review turns up a duplication, an interaction, or a dose that no longer matches what you were told, the finding goes to the doctor who prescribed it, along with a suggested alternative. You get a copy of the same summary.

Once a year is the usual baseline, and it is the standard Medicare Part D applies to eligible members. Sooner is worth it after a hospital stay, after a new specialist adds something, or when a dose changes. Anytime you notice a new symptom that started near a new prescription is also a reason to come in.

Yes. Caregivers are welcome to attend, and many reviews happen with the patient and a family member in the room together. Bring the bottles and the prescriber names even if the patient cannot come. Call first so the pharmacist knows who is coming and what needs to be reviewed.
